How to become a Clyde Disability Services Client

We can work with you to take you through our 3 step process of becoming a Clyde Disability Services client. Our team are here to answer your questions about eligibility.

Are you eligible for disability services?

Eligibility for support services is based on the following criteria:

  • Permanent, or likely to be permanently disabled.
  • Be at least 7 years of age at time of request for service.

Intake Process

Prospective client or family member acts on behalf of the client to commence the intake process. After eligibility is confirmed an intake form is completed. Clyde Support Services Support Co ordinators arrange to meet with the prospective client, either face to face or over video call. Following this meeting a risk assessment is conducted and the intake is finalised. A future needs assessment is conducted before the intake is finalised.
